KKB posting–part 2

       Ok I had just completed my rotation posting in KKB & Serendah…This part of the posting even more exciting than ever…

       On Monday itself we had a super full schedule. We had a talk on the organisation of KKB hospital by Dr. Roosli around 10am. At the end of the talk he told us a news that we had all been waiting forward—there will be a autopsy going to be done that day. We all were so excited when we heard that news. We had a victim who was raped, murdered and burnt. After divided ourselves into groups for usual history taking, we went to forensic straight away. Too bad by the time we reached the police were still doing their investigation. Then we went to do our history taking but every1 just wanted to finished the task as soon as possible and go 4 de post - mortem.

       Around 11am, the body was ready 4 autopsy. We all flocked to the forensic department and waited for our turn to go in. Inside there were a lot of police and pathologist , so to avoid the place from overcrowded we only got 10 mins each person and every time only 4 of us can go in and observed. I waited for about 30mins outside the forensic department. We went in and the pathologist were still taking sample swab from the victim. This victim was burnt so badly until we cant determined her race and her face was disfigured. After 10 mins we were forced to leave the place and let the other ppl to come in. Reluctantly we went for our lunch. Then me, Ben and Mustaq went to the forensic again. This time Sister Rita was not guarding at the entrance and we just went in the room. This round we stayed about an hour inside. We saw the whole process of it–how the body been cut, how the organs were taken out and how to made conclusion such as the time that case happened and how old is the victim based on clinical finding. The pathologist in charged was quite helpful as they willing to teach us and tell us what are the things to look for in each site. She also showed us the decomposed heart and describe the difference between it with normal heart. After seeing the real organs, we can appreciate more about human anatomy.               

        Around 3pm we were brought by Dr Grace to ward 8 for bedside teaching. We were taught about brief history taking and physical examination. Dr Grace showed us a patient with cardiac problem. the patient has fluid accumulation in heart and he presented with dyspnoea on mild exertion. We were taught to do complete examination and all the rational behind every examination. Then Dr Grace showed us another patient with murmur and hypertyroidism. This is the second time i heard a murmur. We were taught how to determine the heart sound by using carotid artery. A lot of things to learnt from Dr Grace and she was a very good teacher. She really knew her stuffs well and can present it in a very confident way. After the session all of us were so impressed and many of the guys even took her as their new idol :P

      We went back our home around 5pm. That night we planned to cook dinner ourselves. Cheemzz brought her multipurpose cooker there and we bought ingredient 4 pasta. Me & Cheemzz started to cook from 7pm and finished around 8.30pm. We cooked pasta 2 packets of pasta and 2 tins og tuna, a packet of nuggets and sausages which were the leftover from BBQ. As the guys in our house were forever in hungry state so we supposed there were nothing left although this was the 1st time i cooked that much of pasta. Cleaned up after the dinner was done by Mustaq this round. Ok i promised Wei Cheong i will give him a vredit in my blog this time. He cleaned up the whole BBQ set that night after me & Cheemzz complaint that he made no contribution at all these 3 days as a imigrant from next door who came to our place to eat.As usual we had mahjong session that night.We then had ice cream as desert around 12am…So crazy la this hour still eat this kind of food, im going to gain lots of weight after this posting :(

        Waked up around 6.40am the next day as we were supposed to go KK Serendah for family medicinine posting. I was so tired that i actually missed my alarm coz the night b4 i chatted with Cheemzz till 4am…After preparing ourselves, we went to Serendah by the bus provided. Reached our destination around 8am. We reported ourselves to Dr Yong Rafidah and again were divided into groups to do our posting. We were supposed to observe what kind of services the KK provide and how the Doc, MA and nurses conduct their duties. We folowed Dr yong to clerk 1 patient. This patient was a hypertensive patient and his BP was 180/120mmHg. This was so high that without any delay Dr Yong sent him for medication and asked him to come back after an hour time. Later we discovered this man was under a lot of family stress. He also had a problem of non- compliance. He did not take his HTN medication regularly as instructed. On examination we found out that his apex was not in the normal position and we suspected he had a left ventricular hypertrophy.We did a fundoscopy on him as we scared he might had a maglinant HTN.

       From the way the patient told us his problem, we can see that stress really was a major aggravating factor to hypertention. As a doctor we can prescribe the best medicine but if we cant deal with the patient psychologically then all the medication we gave will never heal the patient. As in this case we also realised that good doctor-patient relationship is so important.If a doctor can educate a patient well and made them realised the importance of folllowing the medication, the patient will have a higher chances of recovery.

        We went for the Maternal and Child care department after we finished with the HTN case. In there we again gained a lot of new experiences. I saw a PAP smear taken and we asked about the routine that a woman is supposed to do after the aged of 40.Then we saw a pregnant woman and we saw how the nurse do regular antenatal checkup.We had a discussion after the whole day. There we discussed all the cases we observed. Although the whole process was so overwhelming yet we did learn a lot of new stuffs.

        We went for dinner together with group C ppl that day. We were too full after the dinner so instead of taking a ride home we choose to walk home that night.Me, Cheemzz, Ben, Siaw and Mustaq 5 of us went on to explore KKB. We went from street to street and took pictures along the way. The journey was about 30 mins but as we played along the way we didnt feel tired at all. We were back home around 9pm. Then after taking our bathe, me & Cheemzz cooked "tong yuan" as supper that night.We had our tong yuan session about 11.00pm. Continue mahjong and chit-chatting session while the guys do the cleaned up.Afte watching Poseidon till 2am we went to bed ….

          The last day we went Serendah from 8am till 12.30pm. We went to pharmacy and checked out the drugs there. A lot of familiar drugs which i had mostly forgot their uses..Shit need to revise my pharmaco all over again. Then we went to Special Care department. There we saw how the nurse do regular check up for elderly. The nurse interviewed the family member thoroughly as to understand the background and health condition of the elderly. Then later we saw a diabetic patient who had amputated his right leg and right arm. The MA was supposed to do an ECG on him but he faced difficulties as he cant attach the leads on him. We saw how difficult for the patient in this kind of condition and this make us appreciate more about the care for diabetic patients.

       Later we went into OPD and observed the doctor. We saw many cases and how the patient communicate with the patient.The doctor that we followed is a good doctor who can lighten up the atmosphere. He is so energetic and cheerful and this always ease the anxiety of patients. Encounterd some sad cases tho..A pregnant lady was beaten up by her husband and did not seek any help. The doctor referred her to hospital and advised her to lodge a police report.So sad right imagine when u are pregnant and need the most care from family yet u are treated like that..Guys out there…pls be nice to ur wife ok…dont ever ever do this to them!!!!

       We finished the 1 week posting and rush back to kl around 5pm. We went straight to mid valley for movie without going home…Haha happening le…I believed that there is no 1 group as fun as us …We had BBQ, cooking , mahjong, outing and lots of activities thoroughout the posting….Really had a great time and what more can i expect? This is indeed a fruitful experience and thanks to all my gp member esp those stayed in house number 29 for making this whole posting so memorable….

    btw i stayed at 29 too last time. haha. good to know tht u benefited from it. unlike me. haha..
