KKB posting–part 1
Just finished 2 days posting at KKB hospital….Had a great time there and gained quite a lot of new experienced.
We reached KKB around 5 pm on Sunday evening. Then we were given a briefing about our 1 week accommodation by Col Syamsul. Me and Cheemzz went to nursing hostel by bus. Only 2 of us were in the bus as the rest of them are staying at houses allocated and some drives there. We were actually quite reluctant to go there coz we wanted to stay at house.We planned to ask the guys to fetch us back and squeeze with the guys at house if we found the condition at hostel is not ok. Who knows when we reached there, due to error in arrangement there were only 5 beds for 6 of us.Hmm this give us a valid reason to go back the guys place
Upon reaching the guys house, we saw ben alone there and the rest of the guy already went out to cari makan. Ben told us that they had vacant a room for both of us and 4 of them will be sharing a room. So touched huh they actually made the offer…Thanks lo to all the gemtlemen:P After unpacked our stuffs, me and Cheemzz started to clean up the house although the guys said no need as it will be dirty the next day even if we cleaned it up. How can they tolerate the dirty condition and live with it? Sometimes really salute the guys !! After cleaning up Fabian brought us out to restaurant 98 for dinner. As there was only 1 car available we packed food for those who stayed at home. After dinner we went to explore KKB and buy stuffs for the next day’s BBQ.
After taking bath and settle down, we started our 1st mahjong session at KKB. We played until Yih Seong came and shared his experiences in KKB with us. He told us what they had done 4 de past few days and what are the stuffs that we needed to know before we went hospital. Die la after hearing what he said only we realised how unprepared we were. We stopped our mahjong session and started to revise back all our CSU thingy. Studied untill about 1.30pm, we really cannot stand anymore and went to bed.oh ya nearly forgot 1 thing. Our place turned into Genting when YYW lit up 6 pieces of mosquito coil together and smoked the whole house.Luckily there was no fire alarm in the house!!
The next day, me, Cheemzz and Wei loong went market around 6.30 to get chicken for BBQ.Crazy right imagine the 3 of us went market so early to buy vege and chicken. Really felt like auntie le. We went straight to hospital after buying all the stuffs we need. We were given a briefing by Sister Rita , the nurse in charge and Dr.Lim regarding our 3 days schedule and task. Then we were divided into groups to start our history taking. 1 girl and 1 boy paired up so that it is easier for us when we need to examine patient with opposite sex.
Me and YYW went to our assigned patient after the briefing. He was a 20 year old malay male. He was a very cooperative patient and this made my history taking easier. After taking a full history, I performed a physical examination on CVS although this patient was not a CVS patient. I was so anxious when doing the examination as this is the 1st time i do it on real patient. Thank god nothing went wrong and i was able to complete the whole examination.
At the afternoon we were given a bedside teaching by Dr. Sevu. He is very enthusiastic to teach us. He actually called up 1 of his patient and asked the patient to come to the hospital just to show us this CVS case. He asked us to take history and examine the patient then present the case to him.The patient was told not to reveal anything to us as Dr Sevu wanted us to find out the diagnosis our own. The patient said he was perfectly ok when we asked whether he got any complaint about his health. This make things hard for us and we cannot find anything based on the history taking. Then no choice but to proceed physical examination. On inspection, no abnormality was discovered. This patient looks perfectly normal. We went on to auscultation. Wow for the 1st time i heard murmur on real patient so excited. Finally now we know something was wrong in this patient. Now the problem is to identify the underlying illness and made diagnosis. We only know we heard some murmur but none of us can confirm what type of murmur is that. Just as we were busy thinking of al the possibilities, Dr Sevu came and asked us to start our presentation. Along the presentation he told us what are the thing we should be paying attention to in CVS cases and taught us how to related the clinical finding with diagnosis. Then he asked about out diagnosis, but no1 can give a sure answer. Dr. Sevu slowly guided us and helped us to come out with the diagnosis. Finally we came out with a conclusion.The patient had a pansystolic murmur and thus he had a mitral regurgitation.For the 1st time i followed a complete case from history taking until the diagnosis. Really learned a lot from this teaching session.
After the hectic day, we went back to our house to prepare for the BBQ. Of course me and Cheemzz as the only female in the house will prepare the food for the BBQ. The rest of the guys were sleeping while WL made friend with the auntie next door coz we need to borrow tupperware from our neighbour. Me & Cheemzz cleaned up 2 whole chicken, 75 sausages,and 1 packet of fishball. After we marinated the thingy it is almost 6pm. The guys helped to fix up the BBQ pit and lit the fire. Wei Cheong did the"smartest" in the world when they tried to lit the fire without any starter. For all the things available he was so smart to tear the box for BBQ pit to fan the fire. Cant imagine what possessed him and made him do this kind of thing. How are we going to return the BBQ pit with its box torn apart?!?! Beside this strange incident the whole BBQ went out smoothly. We had a great time there. We played, laughed and of course ate a lot until 10 pm. This time the guys did the cleaning and i really need to compliment them for doing a grat job. Our place turned form ashy to clean in a short while.Then again we started mahjong and jim rammy session till 2.00am. Damn havoc la especially some of them who were addicted to mahjong and willing to sacrifices thier sleep for the game.
The next day we went to hospital early in the morning. We witnessed a delivery that day. Although this is not the 1st time i saw a delivery, i still felt excited as before. We witnessed an arrival of a new life around 10.20am. The we saw how the nurse do resuscitation for the baby and how she bathe the baby. The baby was crying so loud when the nurse carry her but when the nurse let her soak in water she immediately stop crying..Haha maybe she likes to swim:P
Then as usual we were supposed to clerk a case and do presentation. We were assigned to female ward that day. Unfortunately our patient was too sick to talk to us. No choice but to approached another patient. The patients there were not as friendly and cooperative compare to the male ward. Maybe because they had been interviewed by the previous groups. Finally me and YYW managed to clerk a malay female. She is the 1st patient i saw with severe pallor. We did physical examination on her and saw the symptom of typical pallor. We started to prepare our presentation after we had our lunch. The teaching session start at 2.15pm. We presented our case to Dr. Lim. She taught us how to get as much information as possible and the correct way to approached a patient. She also taught us how to rule out differential diagnosis by asking relevant question in history taking. Till then only i realised how a simple question which i usually neglect had a great impact on helping doctor to come out with correct diagnosis.
After hearing all the history presented by us, we followed Dr. Lim to the ward.There we learned the way to differentiate conscious and semi-conscious patient, how to palpate for CVS, how to check for peripheral pulses and how to check pedal oedema. She also corrected the way i examined the pallor patient. She taught us how to examined the patient in a more proper way which put priority to make the patient feels comfortable.Before finish our day in hospital, we saw a skin traction performed by MA to a old lady. The lady had a fracture in femur. The MA need to straighten her bending leg by force. The lady was shouting as the pain was so unbearable. After the whole process we had a small chat with the lady. Sad to see this old lady who had neither relatives nor friends.Who is going to take care of her for the rest of her life? I felt kinda sad and helpless as there is nothing much I can do.
About 5pm , we were back at the house again. We packed up our stuffs and prepare to leave KKB for the weekend. We sneaked onto the bus and came back bulit jalil with Group A ppl. Around 7.30pm we were back at KL. Going up to KKB again on Sunday night , I think there will be a lot of new things waiting for us. Looking forward for the coming visit…
