We are always young….

Monday, July 24th, 2006

Wow … just back from YS place which we had our 10 ppl dinner + supper there. Damn full la after eating like 7 or 8 dishes which we cooked since 5 pm to 10.30pm.

Today attended the CSU session with Dr Tin Aung. This is the 1st time i had CSU under him and the 1st thing he taught was medical students always young :)Hehe like to hear that since gals always want to be young n not the other way….

When we 1st enter the medical sch, we are so young compared to the other batches. We look upon to the other seniors and wish to be like them 1 day in the future.

Time flies and now we are going into sem 3. Soon we will be orientating the new batch juniors. Thinking back the days that we were orientated and compare to now, we felt a bit mature. Then again we see the sem 5 ppl graduating from phase 1 we feel humble again..

Sooner or later we will become the sem 5 and graduate from phase 1. We might think we are so great as compare to our juniors. With the ego we go into phase 2. There we will be the youngest again among the ppl in phase 2.

Some time later we will graduate from sem 10 and become a true doctor. With the pride we will start our housemanship. Then we become vy little again in the foodchain of hospital.

Few more years later we will complete the housemanship and de residency. As compare to the new doctors and medical students we are so much higher in rank now. We feel great but hey.. we are still young among all the experienced doctors.

Finally one day we may become a specialist. Now we are so much higher in the foodchain. Even surpass the normal GP. Then just as we feel that we are at the top of the world, something will happen and we just need guidance from senior specialists…Hmm then we feel small again…

True and well said right … Like this speach from Dr Tin Aung. Medicine is a life long learning process. One can never stop learning as long as one is still in this profession.

How much we can learn in our life? We will never know. All we know is we are here to learn as much as possible in order to equip ourself to be a better, more reponsible and more cautious doctor….

Time flies…It had been almost a year ever since i left my home sweet home and come to KL…

In this one yr of Uni life indeed a lot of things had happened. Fortunate to say that many of them are sweet memories. The best thing I own here is my BIG family in IMU. Cant really remember what bring us together, many of us also forgot how we knew each other. Maybe just fate that bring us together and thus the forming of this unique and strong bonding family. There are 12 of us in this family which is a big gp i supposed compared to other small gps in my batch. We have ppl from different age, different background and speak different language.. haha strange right but this is what bring us so much joy…

Let me briefly intro my family.. Ok i will go by chronologic event huh..u guys know what i mean….1st, we have JJ, the big sister in my family. A gal who has the highest pitch and loudest voice in my batch. Always full of confidence and excel in studies. Then we have Ben–the help desk ppl. He is the 1 who we will approached whenever we face problems in studies. .He can solved almost every Q we asked and always can provide us a convincing explanation.

Then is me la nothing much significant but just enjoy being in this happy family. Next is Winnie our dai ga jie..An independent gal who always is the decision maker in our gp. She also is the 1 responsible for our transport.. If not for her, i think i will never go to so many places and had so much fun. Now is my bedmate huan —My foreva friend who is a over-rounded gal. Excel in everything she involved with. A wonderful friend who i had been rely on for almost 10 years…Thanks ya huan ge for being a loyal listener and always by my side whenever i need help. Ok Cheemzz is next. A funky gal with brain…Good in studies as well as sport.. She can play almost every game under the sun. A great cook maybe coz she is from Penang gua..Knowing her more during KKB trip which we had a great time gossiping and playing mahjong….

Then we proceed to our 1 and only siaw. Our guardian angel ever since Sabah trip..Siaw is a genius with photogenic mind..He is a true friend who u can always count on when ever u need help.( this make him so important ..right siaw??) Getting to know this mysterious legend more and more..The more i know the more impressed i felt for this little boy..The next is Chowchow( the best??)..A guy o should i say a boy who likes to sing and can sing with 100% passion.. We call him auntie killer as he is the type of mummy’s boy which every aunties adore..Chow is our IT ppl and everything related to it we will ask him for help..Oh ya Chow oso is the 1 who likes everything sweet and cannot live without them…Seldom see guys so addicted to sweet thingy de le…Be carefull ya chow..risk factor 4 DM a…

Then we proceed to hui lin…A gal who always "claim" she is a good listener. But is true oso la since she is the mediator in family who solve all family issues.Had fun with her during the sign language class haha still remember the " poorthing " sign which we keep making fun of it. She is also our flyer maker. Thanks to her we have nice n infomative Hellen Keller flyer…Next i present to u all with tokoh mentakab–Jackie. A responsible guy la i would say…Too responsible in fact..he is a perfectionist who always ended up having too much burden on his shoulder. A guy with his own unique opinion and different way of looking things compare with other guys about his age. Quite a choosy fellow but i think he knows well what he wants la. Looking forward to see his the other half..:P

Who’s next?? Hmm Paul i think…A typical english ed guy who always got so frust when we speak mandarine…Likes to play futsal but always got himself injured…Always hungry and wanted to go mamak…Favourite phrase :" what did i miss "….Ok coming down next is our youngest kid –YYW. A boy who wanted to grow up eagerly and tries vy hard to prove that he is growing up..A helpful friend who is always on call when u need him. He can drive us to whichever place we wanted to go and help us to do whatever thing we request. Haha really admire him when he open up my room for me when i left my key inside the room n lock the door up. Haha…Still remember he say " anything happen just count on me la" ..YYW, u r a great guy so just have faith in urself and im sure u can find some1 at the end of the day de..not to worry since u ‘r still young wat!!

Above are my family members who i cherish a lot..Because of them i know i will never be alone in this foreign land..approximately 300++km away from my dearest family at JB. Thanks them lots for giving me the sense of belongingness..Really if not because of u all i think i wont be able to enjoy Uni life that much esp in such a stressful place–IMU. I know i will never be alone on the way of pursuing my dreamzz…U guys really meant a lot to me…Love ya!

KKB posting–part 2

Wednesday, July 19th, 2006

Ok I had just completed my rotation posting in KKB & Serendah…This part of the posting even more exciting than ever…

On Monday itself we had a super full schedule. We had a talk on the organisation of KKB hospital by Dr. Roosli around 10am. At the end of the talk he told us a news that we had all been waiting forward—there will be a autopsy going to be done that day. We all were so excited when we heard that news. We had a victim who was raped, murdered and burnt. After divided ourselves into groups for usual history taking, we went to forensic straight away. Too bad by the time we reached the police were still doing their investigation. Then we went to do our history taking but every1 just wanted to finished the task as soon as possible and go 4 de post - mortem.

Around 11am, the body was ready 4 autopsy. We all flocked to the forensic department and waited for our turn to go in. Inside there were a lot of police and pathologist , so to avoid the place from overcrowded we only got 10 mins each person and every time only 4 of us can go in and observed. I waited for about 30mins outside the forensic department. We went in and the pathologist were still taking sample swab from the victim. This victim was burnt so badly until we cant determined her race and her face was disfigured. After 10 mins we were forced to leave the place and let the other ppl to come in. Reluctantly we went for our lunch. Then me, Ben and Mustaq went to the forensic again. This time Sister Rita was not guarding at the entrance and we just went in the room. This round we stayed about an hour inside. We saw the whole process of it–how the body been cut, how the organs were taken out and how to made conclusion such as the time that case happened and how old is the victim based on clinical finding. The pathologist in charged was quite helpful as they willing to teach us and tell us what are the things to look for in each site. She also showed us the decomposed heart and describe the difference between it with normal heart. After seeing the real organs, we can appreciate more about human anatomy.

Around 3pm we were brought by Dr Grace to ward 8 for bedside teaching. We were taught about brief history taking and physical examination. Dr Grace showed us a patient with cardiac problem. the patient has fluid accumulation in heart and he presented with dyspnoea on mild exertion. We were taught to do complete examination and all the rational behind every examination. Then Dr Grace showed us another patient with murmur and hypertyroidism. This is the second time i heard a murmur. We were taught how to determine the heart sound by using carotid artery. A lot of things to learnt from Dr Grace and she was a very good teacher. She really knew her stuffs well and can present it in a very confident way. After the session all of us were so impressed and many of the guys even took her as their new idol :P

We went back our home around 5pm. That night we planned to cook dinner ourselves. Cheemzz brought her multipurpose cooker there and we bought ingredient 4 pasta. Me & Cheemzz started to cook from 7pm and finished around 8.30pm. We cooked pasta 2 packets of pasta and 2 tins og tuna, a packet of nuggets and sausages which were the leftover from BBQ. As the guys in our house were forever in hungry state so we supposed there were nothing left although this was the 1st time i cooked that much of pasta. Cleaned up after the dinner was done by Mustaq this round. Ok i promised Wei Cheong i will give him a vredit in my blog this time. He cleaned up the whole BBQ set that night after me & Cheemzz complaint that he made no contribution at all these 3 days as a imigrant from next door who came to our place to eat.As usual we had mahjong session that night. We then had ice cream as desert around 12am…So crazy la this hour still eat this kind of food, im going to gain lots of weight after this posting :(

Waked up around 6.40am the next day as we were supposed to go KK Serendah for family medicinine posting. I was so tired that i actually missed my alarm coz the night b4 i chatted with Cheemzz till 4am…After preparing ourselves, we went to Serendah by the bus provided. Reached our destination around 8am. We reported ourselves to Dr Yong Rafidah and again were divided into groups to do our posting. We were supposed to observe what kind of services the KK provide and how the Doc, MA and nurses conduct their duties. We folowed Dr yong to clerk 1 patient. This patient was a hypertensive patient and his BP was 180/120mmHg. This was so high that without any delay Dr Yong sent him for medication and asked him to come back after an hour time. Later we discovered this man was under a lot of family stress. He also had a problem of non- compliance. He did not take his HTN medication regularly as instructed. On examination we found out that his apex was not in the normal position and we suspected he had a left ventricular hypertrophy. We did a fundoscopy on him as we scared he might had a maglinant HTN.

From the way the patient told us his problem, we can see that stress really was a major aggravating factor to hypertention. As a doctor we can prescribe the best medicine but if we cant deal with the patient psychologically then all the medication we gave will never heal the patient. As in this case we also realised that good doctor-patient relationship is so important. If a doctor can educate a patient well and made them realised the importance of folllowing the medication, the patient will have a higher chances of recovery.

We went for the Maternal and Child care department after we finished with the HTN case. In there we again gained a lot of new experiences. I saw a PAP smear taken and we asked about the routine that a woman is supposed to do after the aged of 40.Then we saw a pregnant woman and we saw how the nurse do regular antenatal checkup. We had a discussion after the whole day. There we discussed all the cases we observed. Although the whole process was so overwhelming yet we did learn a lot of new stuffs.

We went for dinner together with group C ppl that day. We were too full after the dinner so instead of taking a ride home we choose to walk home that night.Me, Cheemzz, Ben, Siaw and Mustaq 5 of us went on to explore KKB. We went from street to street and took pictures along the way. The journey was about 30 mins but as we played along the way we didnt feel tired at all. We were back home around 9pm. Then after taking our bathe, me & Cheemzz cooked "tong yuan" as supper that night. We had our tong yuan session about 11.00pm. Continue mahjong and chit-chatting session while the guys do the cleaned up.Afte watching Poseidon till 2am we went to bed ….

The last day we went Serendah from 8am till 12.30pm. We went to pharmacy and checked out the drugs there. A lot of familiar drugs which i had mostly forgot their uses..Shit need to revise my pharmaco all over again. Then we went to Special Care department. There we saw how the nurse do regular check up for elderly. The nurse interviewed the family member thoroughly as to understand the background and health condition of the elderly. Then later we saw a diabetic patient who had amputated his right leg and right arm. The MA was supposed to do an ECG on him but he faced difficulties as he cant attach the leads on him. We saw how difficult for the patient in this kind of condition and this make us appreciate more about the care for diabetic patients.

Later we went into OPD and observed the doctor. We saw many cases and how the patient communicate with the patient. The doctor that we followed is a good doctor who can lighten up the atmosphere. He is so energetic and cheerful and this always ease the anxiety of patients. Encounterd some sad cases tho..A pregnant lady was beaten up by her husband and did not seek any help. The doctor referred her to hospital and advised her to lodge a police report. So sad right imagine when u are pregnant and need the most care from family yet u are treated like that..Guys out there…pls be nice to ur wife ok…dont ever ever do this to them!!!!

We finished the 1 week posting and rush back to kl around 5pm. We went straight to mid valley for movie without going home…Haha happening le…I believed that there is no 1 group as fun as us …We had BBQ, cooking , mahjong, outing and lots of activities thoroughout the posting….Really had a great time and what more can i expect? This is indeed a fruitful experience and thanks to all my gp member esp those stayed in house number 29 for making this whole posting so memorable….

KKB posting–part 1

Saturday, July 15th, 2006

Just finished 2 days posting at KKB hospital….Had a great time there and gained quite a lot of new experienced.

We reached KKB around 5 pm on Sunday evening. Then we were given a briefing about our 1 week accommodation by Col Syamsul. Me and Cheemzz went to nursing hostel by bus. Only 2 of us were in the bus as the rest of them are staying at houses allocated and some drives there. We were actually quite reluctant to go there coz we wanted to stay at house. We planned to ask the guys to fetch us back and squeeze with the guys at house if we found the condition at hostel is not ok. Who knows when we reached there, due to error in arrangement there were only 5 beds for 6 of us.Hmm this give us a valid reason to go back the guys place :)

Upon reaching the guys house, we saw ben alone there and the rest of the guy already went out to cari makan. Ben told us that they had vacant a room for both of us and 4 of them will be sharing a room. So touched huh they actually made the offer…Thanks lo to all the gemtlemen:P After unpacked our stuffs, me and Cheemzz started to clean up the house although the guys said no need as it will be dirty the next day even if we cleaned it up. How can they tolerate the dirty condition and live with it? Sometimes really salute the guys !! After cleaning up Fabian brought us out to restaurant 98 for dinner. As there was only 1 car available we packed food for those who stayed at home. After dinner we went to explore KKB and buy stuffs for the next day’s BBQ.

After taking bath and settle down, we started our 1st mahjong session at KKB. We played until Yih Seong came and shared his experiences in KKB with us. He told us what they had done 4 de past few days and what are the stuffs that we needed to know before we went hospital. Die la after hearing what he said only we realised how unprepared we were. We stopped our mahjong session and started to revise back all our CSU thingy. Studied untill about 1.30pm, we really cannot stand anymore and went to bed.oh ya nearly forgot 1 thing. Our place turned into Genting when YYW lit up 6 pieces of mosquito coil together and smoked the whole house.Luckily there was no fire alarm in the house!!

The next day, me, Cheemzz and Wei loong went market around 6.30 to get chicken for BBQ.Crazy right imagine the 3 of us went market so early to buy vege and chicken. Really felt like auntie le. We went straight to hospital after buying all the stuffs we need. We were given a briefing by Sister Rita , the nurse in charge and Dr.Lim regarding our 3 days schedule and task. Then we were divided into groups to start our history taking. 1 girl and 1 boy paired up so that it is easier for us when we need to examine patient with opposite sex.

Me and YYW went to our assigned patient after the briefing. He was a 20 year old malay male. He was a very cooperative patient and this made my history taking easier. After taking a full history, I performed a physical examination on CVS although this patient was not a CVS patient. I was so anxious when doing the examination as this is the 1st time i do it on real patient. Thank god nothing went wrong and i was able to complete the whole examination.

At the afternoon we were given a bedside teaching by Dr. Sevu. He is very enthusiastic to teach us. He actually called up 1 of his patient and asked the patient to come to the hospital just to show us this CVS case. He asked us to take history and examine the patient then present the case to him.The patient was told not to reveal anything to us as Dr Sevu wanted us to find out the diagnosis our own. The patient said he was perfectly ok when we asked whether he got any complaint about his health. This make things hard for us and we cannot find anything based on the history taking. Then no choice but to proceed physical examination. On inspection, no abnormality was discovered. This patient looks perfectly normal. We went on to auscultation. Wow for the 1st time i heard murmur on real patient so excited. Finally now we know something was wrong in this patient. Now the problem is to identify the underlying illness and made diagnosis. We only know we heard some murmur but none of us can confirm what type of murmur is that. Just as we were busy thinking of al the possibilities, Dr Sevu came and asked us to start our presentation. Along the presentation he told us what are the thing we should be paying attention to in CVS cases and taught us how to related the clinical finding with diagnosis. Then he asked about out diagnosis, but no1 can give a sure answer. Dr. Sevu slowly guided us and helped us to come out with the diagnosis. Finally we came out with a conclusion. The patient had a pansystolic murmur and thus he had a mitral regurgitation. For the 1st time i followed a complete case from history taking until the diagnosis. Really learned a lot from this teaching session.

After the hectic day, we went back to our house to prepare for the BBQ. Of course me and Cheemzz as the only female in the house will prepare the food for the BBQ. The rest of the guys were sleeping while WL made friend with the auntie next door coz we need to borrow tupperware from our neighbour. Me & Cheemzz cleaned up 2 whole chicken, 75 sausages, and 1 packet of fishball. After we marinated the thingy it is almost 6pm. The guys helped to fix up the BBQ pit and lit the fire. Wei Cheong did the"smartest" in the world when they tried to lit the fire without any starter. For all the things available he was so smart to tear the box for BBQ pit to fan the fire. Cant imagine what possessed him and made him do this kind of thing. How are we going to return the BBQ pit with its box torn apart?!?! Beside this strange incident the whole BBQ went out smoothly. We had a great time there. We played, laughed and of course ate a lot until 10 pm. This time the guys did the cleaning and i really need to compliment them for doing a grat job. Our place turned form ashy to clean in a short while. Then again we started mahjong and jim rammy session till 2.00am. Damn havoc la especially some of them who were addicted to mahjong and willing to sacrifices thier sleep for the game.

The next day we went to hospital early in the morning. We witnessed a delivery that day. Although this is not the 1st time i saw a delivery, i still felt excited as before. We witnessed an arrival of a new life around 10.20am. The we saw how the nurse do resuscitation for the baby and how she bathe the baby. The baby was crying so loud when the nurse carry her but when the nurse let her soak in water she immediately stop crying..Haha maybe she likes to swim:P

Then as usual we were supposed to clerk a case and do presentation. We were assigned to female ward that day. Unfortunately our patient was too sick to talk to us. No choice but to approached another patient. The patients there were not as friendly and cooperative compare to the male ward. Maybe because they had been interviewed by the previous groups. Finally me and YYW managed to clerk a malay female. She is the 1st patient i saw with severe pallor. We did physical examination on her and saw the symptom of typical pallor. We started to prepare our presentation after we had our lunch. The teaching session start at 2.15pm. We presented our case to Dr. Lim. She taught us how to get as much information as possible and the correct way to approached a patient. She also taught us how to rule out differential diagnosis by asking relevant question in history taking. Till then only i realised how a simple question which i usually neglect had a great impact on helping doctor to come out with correct diagnosis.

After hearing all the history presented by us, we followed Dr. Lim to the ward. There we learned the way to differentiate conscious and semi-conscious patient, how to palpate for CVS, how to check for peripheral pulses and how to check pedal oedema. She also corrected the way i examined the pallor patient. She taught us how to examined the patient in a more proper way which put priority to make the patient feels comfortable. Before finish our day in hospital, we saw a skin traction performed by MA to a old lady. The lady had a fracture in femur. The MA need to straighten her bending leg by force. The lady was shouting as the pain was so unbearable. After the whole process we had a small chat with the lady. Sad to see this old lady who had neither relatives nor friends.Who is going to take care of her for the rest of her life? I felt kinda sad and helpless as there is nothing much I can do.

About 5pm , we were back at the house again. We packed up our stuffs and prepare to leave KKB for the weekend. We sneaked onto the bus and came back bulit jalil with Group A ppl. Around 7.30pm we were back at KL. Going up to KKB again on Sunday night , I think there will be a lot of new things waiting for us. Looking forward for the coming visit…

How to deal with angry patient!?!?!

Tuesday, July 11th, 2006

Just had a behavioural sciences lesson which is about the way of dealing angry patients.

We were given 4 scenario which there will be stimulated patient showing their anger and bombarded us with all sort of emotion…

We started with a milder dose, the SP was a little bit angry but still can be consider cooperative. So there were not much problem for us to cope and handle the situation. 3 of us tried on to interviewed the SP and we got very useful feedback from our facilitator– Mr Galy.

Then as we were more "experienced", we get another SP who appeared to be angrier than the previous 1. 3 of us tried to interviewed as well this time seem to be more challenging and some of us got stuck and cant continue the conversation.

After finished with the 2 SP , Mr Galy introduced us to the 3rd SP. And now we had Mustaq as de "doctor"..The fun part begun….The SP is really a well trained 1.. He really can act damn well. He was so angry and frustrated and keep scolding Mustaq for the hurdle he had gone thorugh before actually seeing the doctor. He was cursing the nurses who ran him to polyclinic instead of A&E as he was having an asthmatic attack. Mustaq was totally unable to calm down the patient. The SP keep saying he is short of breath and refuse to answer any question Mustaq asked. Damn funny la when the SP keep panting and Mustaq keep asking questions.Haha… This is how the patient react : I had told u i am SOB, y u keep asking my question!!! Cant u just put me in nebulliser den i can answer u 101 question after this.." Imagine ur patient telling u this …wah really dunno how to react!! pengsan wei..

The atmosphere was so intense and every 1 was waiting for Mustaq response…And finally.. Mustaq said:" I really cant proceed any more" Wahaha every 1 burst and laugh so hard even Mr Galy did so too. Then the SP started to explained wat knid of response he wanted from us… Luckily Mr Galy was not vy upset with our performance and he said we still "young" so is ok we feel panic when handle this kind of situation…..

Wah really learn a lot during this session. It really need a lot of skill and practice in order to deal with such difficult patients…Cant imagine if 1 day in real life when i really become a doctor and got stuck in this kind of situation. Communication skill really essential for us as we are dealing with individual who are all different and may react differently to certain scenario. Hmm better start to buck up in this thing before i become a true doctor.. I certainy wouln’t want to surrender in front of my patient in the future and say " look u are too difficult to handle, could u pls come urself down before i start to examine u!!" This obviously not going to work in real life….

Still lot more things to learn and lot more skills to acquire….Medical profession really is something chanllenging and interesting..Starting to like this thing more and more ( beside the never endless exam) …Tomolo going Kuala Kurbu Baru Hospital for rotation lo…Hopefully can get thro this thingy without landed in a mess ….
